Medvedev described von der Leyen as an "old hag" and a "mad gynaecologist", and accused her of putting Kyiv's interests ahead of those of Europe.
The remarks are part of a broader pattern of hostile Russian rhetoric toward von der Leyen. Russian officials and state media have repeatedly criticised her over her support for Ukraine, European defence policy and sanctions against Moscow.
Von der Leyen has continued to advocate European support for Ukraine and maintaining pressure on Russia. In June, she said the EU had agreed to extend Russian sanctions for 12 months and was continuing work on another sanctions package.
